Voldemort's Name

The name "Lord Voldemort" is a pseudonym that Tom Marvolo Riddle creates during his time as a student at Hogwarts. It is an anagram of his real name; "Tom Marvolo Riddle" can be rearranged to spell "I am Lord Voldemort". Voldemort's teenage memory describes his motives for the name change: "You think I was going to use my filthy Muggle father's name forever? [...] No, Harry—I fashioned myself a new name, a name I knew wizards everywhere would one day fear to speak, when I had become the greatest sorcerer in the world!" (CS17).
Lord Voldemort is most often referred to as "You-Know-Who" or "He-Who-Must-Not-Be-Named"; during his first reign of terror over the British wizarding population, the practice of being afraid of even speaking his name became widespread, persisting even years after his downfall.
Death Eaters usually refer to him as "the Dark Lord", and do not tolerate it when undeserving wizards befoul the name "Voldemort". Bellatrix reprimands Harry for doing so during the Battle of the Department of Mysteries (OP35). Likewise, Snape lectures Harry for speaking the name, pointing out that while Dumbledore is powerful enough to have that right, Harry is far from that.
Albus Dumbledore calls him "Tom" when they duel, irritatingly highlighting both his part-Muggle ancestry and his common, unremarkable real name (OP36). Albus claims that that is due to the memories of teenager Riddle at school when he first talks to Voldemort after the many years the wizard spent away in deep researches involving dark magic (HBP20). Dumbledore has called him "Tom" ever since they met, when the future Dark Lord was only a child about to start Hogwarts (HBP13).
Some members of the Order of the Phoenix call him "Lord Voldemort", including Remus (PA8), Sirius (GF27), and McGonagall (albeit grudgingly; PS1). Hagrid only does so when Harry's ignorance of the wizarding world forces the half-giant to tell him about the Dark Lord (PS4).
Harry at first refrained from referring to Voldemort as such when in company of people who request for him not to do so, such as Hagrid (PS5) and Ron (PS6); Dumbledore reprimanded him for calling Voldemort "You-Know-Who" at the end of the boy's first year at Hogwarts, pointing out that "fear of a name only increases fear of the thing itself" (PS17).
