A Portkey is an object that has been enchanted to transport anyone who touches it to a specific place. Usually, portkeys are useless Muggle trash, to be sure that Muggles won't pick them up and accidentally transport themselves to somewhere else.
Portkeys have to be authorised by the Ministry, otherwise the person responsible will be severly punished. In Harry Potter and the Order of the Phoenix, Dumbledore set up an unathourised portkey in the Ministry of Magic in front of Fudge's nose but wasn't punished.
In Harry Potter and the Goblet of Fire, Barty Crouch Jr., disguised as Alastor Moody, turned the Triwizard Cup into a Portkey so as to take Harry to the graveyard where Lord Voldemort and Wormtail waited for him.
