The Azkaban fortress is the wizard prison, to where the worst dark wizards and those who break wizard law in the most severe ways are sent. Dementors guard it.

Azkaban is located in a solitary, small island in the North Sea. It is a terrible place, feared by all wizards. Dementors create a miserable atmosphere, intended to provoke desperation and drive prisoners insane. There, convicts are tortured and haunted by their most dreadful deeds; no positive thoughts or happy memories remain in their minds, since the creatures feed on those.

Most prisoners go insane; many of them die and are buried inside the prison (HBP17). The only known wizard to have escaped the prison is Sirius Black, the alledged murderer of Peter Pettigrew. Sirius declares that he managed not to go mad by clinging to the certainty of his innocence, which wasn't a positive thought, and therefore couldn't be absorbed by the Dementors (GF27).

Bartemius Crouch, Jr. also left Azkaban with the help of his parents, but that fact is not common knowledge in the wizarding world. His mother took pity on him, and used a Polyjuice Potion to impersonate her son, who then left the prison accompanied by his father, Bartemius Crouch, Sr. (GF35). This eventually allowed Barty Jr. to break free of his father's tight leash on him and impersonate Alastor Moody as a Hogwarts professor.

First breakout

Albus Dumbledore had repeatedly warned the Ministry of the possibility that Dementors would join Lord Voldemort (GF36). Cornelius Fudge, who was Minister for Magic at the time, paid no attention to the Hogwarts Headmaster's pleas. When the event took place, a mass breakout occurred, and ten dangerous Death Eaters were allowed to escape. The Ministry offered a thousand-Galleon reward for any information that might lead to the escapees (OP25).

Some of the fugitives were recaptured, such as Antonin Dolohov, Rabastan Lestrange and Augustus Rookwood (OP35).

Second breakout

Another breakout occurred in the summer of 1997; it was covered up by the Ministry. Death Eaters such as Lucius Malfoy and Travers escaped; so did Stan Shunpike, who was placed under the Imperius Cursed (DH4, DH5).
